• Duly filled passport application form in Black ink

Requirements for Minor passports

• Copy of parents’ passports (Page 1).
• Two passport size photographs(coloured on a white background).
• Duly filled two passport application forms in Black ink.
• Duly filled Consent form.
• Pricelist available under Quick links
• Applicants to present themselves in person
