Authority letter to import human remains

Requirements for Issuance of Authority letter to import human remains from accredited countries into the Republic of Malawi.

Submit the following:

1. Death Certificate
2. Embalming certificate
3. Deceased passport/police affidavit in the absence of a passport
4. Authority letter to export human remains (from accredited countries’ Government Health Department).
5. Non Epidemic Area Certificate/Non infectious disease
6. Details of the Consigner
7. Consignee details; Full name, Address and Phone numbers.
8. Physical Home address of the deceased; village, Traditional Authority and District.
